Usporedba učinka terapije udarnim valom i terapije ultrazvukom i laserom na funkcionalnost ramena kod kalcificiranog tendinitisa
The effect of shock wave therapy in relation to therapy of therapeutic ultrasound and laser for calcified shoulder tendinitis
Sažetak
Uvod: Kalcificirajući tendinitis rotatorne manšete čest je poremećaj koji izaziva bolnost i ograničenja pokreta u ramenu, a samim time utječu i na obavljanje svakodnevnih aktivnosti. Cilj ovog istraživanja je usporediti učinak radijalnog udarnog vala s terapijskim ultrazvukom i laserom na funkciju, pokretljivost i bol kod oboljelih pacijenata.
Materijali i metode: Istraživanje provedeno na ispitanicima s potvrđenom dijagnozom kalcificirajućeg tendinitisa m. supraspinatusa provedeno je u KB „Sveti Duh“ i trajalo je od rujna 2024. do veljače 2025. godine. U eksperimentalnoj skupini ispitanicima se aplicirao udarni val uz izometričke vježbe snage. U kontrolnoj skupini se ispitanicima aplicirao terapijski ultrazvuk i laser uz izometričke vježbe snage. Provedena su mjerenja opsega pokreta u ramenom zglobu, mjerenja mišićne snage, upitnik o boli i funkcionalnoj sposobnosti ramena (SPADI), te procjena intenziteta boli putem vizualno analogne skale (VAS).
Rezultati: Rezultati su pokazali statistički značajnu razliku među promatranim metodama, pri čemu je terapija radijalnim udarnim valom dovela do značajnog povećanja opsega pokreta i smanjenja boli kod pacijenata u usporedbi s terapijom terapijskim ultrazvukom i terapijskim laserom.
Zaključak: Terapijom radijalnim udarnim valom postižu se bolji rezultati kod opsega pokreta ramenog zgloba i smanjenja boli u usporedbi s terapijom ultrazvukom i laserom u skupini bolesnika s kalcificirajućim tendinitsom. Potrebno je provesti daljnja istraživanja na većem uzorku ispitanika.

Abstract
Introduction: Calcifying rotator cuff tendinitis is a common disorder that causes pain and limited movement in the shoulder, thus affecting the performance of daily activities. The aim of this study is to compare the effect of radial shockwave therapy with therapeutic ultrasound and laser on function, mobility and pain in affected patients.
Subjects and methods: The research conducted on subjects with a confirmed diagnosis of calcific tendinitis of the supraspinatus muscle was conducted in the Sveti Duh Hospital and lasted from September 2024 to February 2025. In the experimental group, the subjects were administered a shock wave along with isometric strength exercises. In the control group, subjects were administered therapeutic ultrasound and laser along with isometric strength exercises. Measurements of range of motion in the shoulder joint, measurements of muscle strength, shoulder pain and functional capacity questionnaire (SPADI), and assessment of pain intensity using a visual analog scale (VAS) were performed.
Results: The results showed a statistically significant difference between the observed methods, with radial shockwave therapy leading to a significant increase in range of motion and a decrease in pain in patients compared to therapeutic ultrasound and therapeutic laser therapy.
Conclusion: Radial shock wave therapy achieves better results in shoulder joint range of motion and pain reduction compared to ultrasound and laser therapy in a group of patients with calcific tendinitis. Further research is needed on a larger sample of subjects.
